Digital Forms

FUHSD HAS PARTNERED WITH CHALK SCHOOLS TO PROVIDE DIGITAL FORMS TO PARENTS, STUDENTS AND STAFF! 

We are excited to provide a new service to you by making forms much easier to complete!  Now you can sign and submit paperwork easily from your computer, smartphone, tablet, or any electronic device.  The system is fast, efficient and highly secure. 

Starting this year, we will be using Chalk Forms to distribute digital forms for field trips, athletics, clubs, registration, Human Resources and many others we previously had on paper.  With over 150 different forms used in the district, this will be a gradual process as we convert forms from paper to paperless throughout the year.

You won’t need to sign up anywhere or get an account in order complete these forms. Online links will be distributed by email and on our websites so you can access them conveniently from anywhere.

Chalk Forms are part of our ongoing initiative to leverage effective and efficient applications of technology in our educational programs and daily operations. We’re committed to providing innovative services and building a community where every resource can be directed toward student success. These forms will help us increase capacity by going paperless and we’re excited to save you time and trouble while also saving trees!

Why Use Digital Forms?

There are many advantages to using digital/electronic forms instead of paper forms.  A few highlights are included below:

Time
Using digital forms will save the district countless hours every year.  Time spent printing, distributing, collating, mailing, completing, receiving, filing and searching will be cut dramatically.  Time saved will be better spent serving our students, staff and community. 

Cost
Using paper forms can be very expensive and wasteful.  Digital forms save the district money by eliminating/reducing the following costs:
Paper, ink, printing, envelopes, postage, filing, storing and more.

Ease of Use and Convenience
Digital forms are easy and convenient to use.  Users can complete forms on their phone, tablet or computer and can do so anytime and anywhere.  Digital forms also submit to the correct person automatically, eliminating forms mailed or delivered to the wrong person.

Improved Accuracy
Reading hand written forms is very difficult and often creates errors.  Even forms completed on a computer and then printed, requires the data/information gathered to be entered somewhere else.  Digital forms allow information to be used directly in other systems, preventing mistakes caused when re-entering data.

Improved Completion
Digital forms can be set to have required fields that prevent the user from submitting an incomplete form.  Many forms completed by hand are turned in with important information missing. 

Increased Usability of Information/Data
Paper forms are difficult to store, file and retrieve.  With digital forms, data can be retrieved in seconds.  Forms can be searched in countless ways, allowing the District to gather, store and search all forms instantly. No longer are file cabinets, boxes and warehouses needed to store forms. 

Improved Security and Tracking
Every form created and used is digitally tracked, allowing us to know who has viewed it and when. All forms are FERPA and HIPAA compliant ensuring that student/parents rights are protected. All data collected is encrypted before being stored on the Chalk database. Data is transferred securely to and from our servers via industry-standard SSL encryption. Each message sent through the Chalk system is further protected with a personalized token that ensures each recipient is only able to view his or her specific document. 

The cloud application platform and data center operations used by Chalk Schools have been accredited under:

  • ISO 27001
  • SOC 1 and SOC 2/SSAE 16/ISAE 3402
  • PCI Level 1
  • FISMA Moderate
  • Sarbanes-Oxley (SOX)

Up-to-Date E-mail Addresses

WE NEED UP-TO-DATE EMAIL ADDRESSES FOR ALL PARENTS/GUARDIANS

E-mail continues to be a valuable communication tool for our schools and is even more important with our migration to digital forms.

All official student information, including parent e-mail, is stored in our student data system, Infinite Campus (IC).  Infinite Campus provides a parent log-in account that allows parents/guardians to view important information such as grades, attendance and more.

Please log into your parent/guardian account on Infinite Campus and update all parent/guardian e-mail addresses.

If you do not have an Infinite Campus parent account, or are not sure how to log-in, go to your school’s website and look for a symbol like the one below.  From there, you can click on the HELP button. You can also contact your school for help and more information.
