I your student is required to cary and or administer medication at school, please read the District Student Medication Policy before completing the appropriate Authorization form(s):

District Student Medication Policy

In order for students to carry medication on a school campus, the parent/guardian of the student must complete the following form and return it to the District Office for approval by the District Nurse.

Authorization to Carry Medication

In order for students to administer/take medication on a school campus, the parent/guardian of the student must complete the following form and return it to the District Office for approval by the District Nurse.
