Student Medication Forms

I your student is required to carry and or administer medication at school, please read the District Student Medication Policy before completing the appropriate Authorization form(s):

District Student Medication Policy

In order for students to carry medication on a school campus, the parent/guardian of the student must complete the following form and return it to the District Office for approval by the District Nurse.

In order for students to administer/take medication on a school campus, the parent/guardian of the student must complete the following form and return it to the District Office for approval by the District Nurse.
