REGISTRATION & RESIDENCY VERIFICATION
Frequently Asked Questions

Where do I go to register my child for school?
During the annual reregistration period only (January 23 through February 8, 2008), you must go to the school sites on your appointed date and time to reregister your child.  Notices to families involved in reregistration were sent the second week in January.  CLICK here for dates and times.  As a reminder, site reregistration involves current FUHSD 10th grade students and incoming feeder district 8th graders only.

All students whose residency required District approval including those attending FUHSD schools on interdistrict transfers, caregiver affidavits, or other special residency situations must complete reregistration at the Enrollment & Residency Office located at the District Office, 589 W. Fremont Avenue in Sunnyvale.  Notices to families involved in centralized reregistration were sent the second week in January. All other students must complete reregistration at the sites during the reregistration period.

At all other times during the year and for all new incoming students, school registration is processed centrally in the Fremont Union High School District at the Enrollment & Residency Office. Registration is required for students enrolling in the Fremont Union High School District for the first time or returning to the Fremont Union High School District after having withdrawn from school. Proof of District residency or an approved interdistrict attendance agreement is required for all new and students returning to the District after having withdrawn.

Parents/guardians must register their students at:
          Enrollment & Residency Office
Fremont Union High School District Office

589 West Fremont Avenue, Sunnyvale, CA 94087
          Phone: (408) 522-2266

You may register in person Monday-Friday from 8:00AM to 4:00PM. Upon completion of registration, families will be notified of date and time to report to the school site for testing and/or scheduling of classes.

What information do I need to provide in order to register my child for school?
Registration materials are available at the Enrollment & Residency Office. Once your registration is complete, you will be notified of the date and time to report to the school site for testing and/or scheduling of classes.

Complete and return the Student Registration Packet to the Enrollment & Residency Office. This packet (3 pages) includes: 1) Student Registration Form and 2) Emergency and Health Information Form

As a reminder, in addition to the registration forms, you will need to provide:

  1. Proof of Residence documentation. Please bring originals as requested for us to photocopy. You may request to have financial or sensitive information covered prior to copying. Your originals will be immediately returned to you.
  2. Proof of birth date and student’s legal name (certified copy of birth certificate is preferred)
  3. Immunization verification card showing that your child has fulfilled state and county health requirements, including Hepatitis B, Varicella (chicken pox) and a Mantoux Tuberculosis Skin Test.

NOTE: Immunizations and health examinations do not need to be up-to-date in order to BEGIN registration. However, they must be up-to-date and submitted to the Enrollment & Residency Office BEFORE your child may begin school.

If you are registering in advance for Fall 2008, please submit final paperwork no later than two weeks before school begins to ensure school and classroom placement on the first day of school.

If it is determined that false information was given to secure or to determine residency, the student will be disenrolled.

What types of residency documentation do you accept?
Residency documentation assists in establishing that you and your child reside full-time within the Fremont Union High School District boundaries.  You must provide documentation in your name that shows you and your family physically reside full-time at the stated address (primary residence).  Both the name of the parent/guardian and the address must be stated on the documentation presented. Fremont Union High School District accepts the following as verification of district residency:

THREE (3) documents are REQUIRED to establish residency— ONE from each box below.
Post Office boxes do not meet residency requirements.  Only street addresses are accepted.

If a parent and child are living with another family within the District boundaries or renting a room in a residence, documentation requirements still must be met. Contact the Enrollment & Residency Office 408-522-2266 for registration information.

Under certain conditions, Fremont Union High School District accepts interdistrict attendance agreements approved by the district of residence as valid proof of residency. The interdistrict attendance agreement must be approved by Fremont Union High School District BEFORE registration is completed.

What if I don’t have all the documents you require?
Please contact the District Office at (408) 522-2266 so that you may meet with a District administrator regarding your child’s registration.

What grade will you place my child in? May I change that placement?
In keeping with California law, Fremont Union High School District places students in the same grade level they would have attained in the regular course of their educational career. Students entering from out-of-country are placed in the same grade level they would have attained had they attended school in the United States. Grade level acceleration or retention based on English proficiency is not a factor in grade level placement.

If you believe your child’s grade level placement is not appropriate, you must provide the school with documentation to support your request for a change in placement. Please make an appointment to meet with the Guidance Assistant Principal at your child’s school. The final decision regarding grade placement rests with the school.

We’re moving and my child will be a senior next year. Is s/he eligible for Senior Privilege?
To request senior privilege, a student must

  • be currently enrolled in and attending a Fremont Union High School District school
  • have a pattern of on-time, regular school attendance and satisfactory school behavior
  • be eligible for graduation with his/her class in June of 2009
  • have moved from his/her current residence after May 15, 2008. Documentation supporting the date of move is required.

All requests for senior privilege are handled at the Enrollment & Residency Office. Students who have moved from one residence within the District to another residence within the District will be granted Senior Privilege as long as the student meets the conditions stated above and has provided documentation which supports the date of move.

Students who have moved outside District boundaries may also be eligible for Senior Privilege. In accordance with our attendance agreement with districts within Santa Clara County, if a student is released from the district of residence, we will grant the interdistrict transfer request as long as the student meets the conditions stated above and has provided documentation which supports the date of move.

As with all interdistrict transfers, students must be in good academic standing, have on-time, regular attendance, and have demonstrated appropriate school behavior.

If it is determined that false information was given to secure or to determine residency, the student will be disenrolled.

How do I obtain an Interdistrict Transfer?
Transferring into the Fremont Union High School District: Students who wish to transfer to Fremont Union High School District should contact their district of residence and file an interdistrict transfer. Transfers must first be approved by the resident district. Due to space and enrollment considerations, Fremont Union High School District limits the number of incoming interdistricts.

Students on interdistrict attendance agreements must exhibit satisfactory attendance, academic progress, and behavior in order to be accepted by and remain enrolled in the Fremont Union High School District.

Transportation is not provided for students attending Fremont Union High School District on an interdistrict transfer. Transportation is the responsibility of the parent/guardian.

Transferring out of the Fremont Union High School District: Students who live within the boundaries of the Fremont Union High School District and would like to attend school in another district may apply for an interdistrict transfer. The Interdistrict Transfer form is available at the District Office. Transfers must be approved by the Fremont Union High School District before submission to the district requested.

What is a Caregiver Affidavit?
Do students living with their parents need to file a Caregiver Affidavit?
In general terms, Caregivers are individuals who provide FULL-TIME care and support to a child whose parents are unable to do so because of incarceration, serious illness, or other hardship. Caregivers are usually relatives of the child’s parent(s). Those not related to the student may be required to apply for a foster care license.

The intent of the Caregiver Affidavit was to provide Caregivers with a means for enrolling students in school in cases where the parents were incarcerated, seriously ill, or experiencing some other difficulty or hardship. It was never the intent of the Caregiver legislation for the affidavit to be used as solely a means to secure District residency for students whose parents do not reside within District boundaries. Such intent is a fraudulent use of the Affidavit and the law provides for prosecution of such violators.

Under District regulation, students shall qualify as District residents if they reside FULL-TIME in the home of a care-giving adult within District boundaries. As with all District residents, Caregivers must provide residency verification as detailed previously.

“FULL-TIME” for the purposes of this regulation is defined as:
               1. The student’s primary residence is that of the caregiving adult, 24 hours a day, seven days a week.
               2. The student resides in the home of the caregiving adult during periods of vacation and/or repose.

Caregiver Affidavits are available at the Enrollment & Residency Office. As stated on the Affidavit, the student must be currently residing with the Caregiver or enrollment will be denied by the District. Students and their caregivers are required to meet with District administrators prior to approval of enrollment. The Caregiver is required to state under penalty of perjury that all information provided is true and correct. If it is determined that false information was given to secure or to determine residency, the student will be disenrolled; the Caregiver may face prosecution.

We are in the process of moving from one home to another.  Our new home is in the District.
How do I go about verifying my residency?

Residency within the Fremont Union High School District is based on the location of your primary residence.   Because you may only have one residence for the purposes of establishing residency and you and your family must physically reside within District boundaries, residency verification documentation is required.  However, since documentation can often be delayed when moving from one home to another, we are frequently able to assist families by making home visits to one or both locations to assist in verifying District residency.

Whether your child is a newly enrolled or continuing student, it is always best to alert us to your move before it happens.  Because the date of move may affect your child’s placement, it is best that you contact us as soon as your move date is confirmed.  The most common situations are listed below.  Rest assured that we want to work with you to ensure a smooth transition.   Please contact our Central Enrollment Desk at (408) 522-2266 and speak with one of our staff members as soon as you have confirmation of your date of move.

Moves involving rental properties:  If you will be renting a new home/apartment, you will need to bring in your new rental or lease agreement.  The agreement must note your dates of occupancy, name the occupants (including the student) and your move-in date. Finally, since it will be difficult for you to provide your change of address on documents listed in #2 above, we will either request alternate documents or work with you to make home visit to assist in verifying your residency.

Moves involving the purchase of a new home:  If you are purchasing a new home, due to recording and closing requirements, you will be unable to provide the property tax statement listed above. Frequently the occupancy of your new home may be delayed because of rent-back agreements, final walk-throughs, and final closing on the sale of the property. The situations must be documented as part of the escrow process.  Since each case is different, we will work with you in helping you to obtain appropriate documentation from your title company or real estate office as well as arranging a home visit to your new residence to assist in verifying your residency.
